How do your teachers import/migrate grades for a student who transfers to his/her course?

We often have students who transfer from one class to another.  I know how to export grades from one Canvas course and import to another Canvas course using a .CSV file. However, it becomes tricky when let's say, a student transfers from History section 1 to History section 2 course which has two different teachers, and their assignments are not necessarily the same.  How do you and your teachers deal with this type of situation in an efficient manner and at the same time, trying to get complete and accurate data for the student who transferred?  I know there's no perfect solution, yet I'm still curious as to what others do in this type of situation.
