Canvas Studio Quizzes won't load properly

We have a case where a teacher is using video quizzes, but is having some issues this semester. What is happening is the quiz won't load on "top" of the embedded YouTube clip. Here's an example.
From the teacher:
1. I have changed nothing, from the 1st semester when there were no problems, to this semester.
2. Monday about 50% of my students in two classes could answer questions and submit scores. Today only a couple could, Same test. Same class. Same everything.
3. Last week the results were different in 2nd hour, v 6th hour, v 8th hour - all in the same day.
Has anyone else experienced this?
