Edit "Report a Problem" Form?

Is there a way to edit the content on the "Report a Problem" form?  We would like to add fields to help us collect all the data we need to troubleshoot the problem (e.g. username, course number, etc.).

As far as I know, there's no way to change it.  However, I'd suggest reaching out to your CSM to confirm that.
