Formula Questions

Can I create a formula questions that has two answers?   I am looking to create a questions where the students convert rectangular coordinates to polar coordinates.  

I am starting out with ([x}, [y]) and want the students to answer with (r, degrees)

For example,  (4,3) would net an answer of (5, 36.9)
