Images disappearing

Why do the images I add to quizzes keep disappearing ?  It is very frustrating.

When I upload the image comes up with a question mark like it doesn't exist. Random older images are like this too - question mark like it can't find the image.

Hi, podols and  @cclowes . This is something that can happen from time to time for various reasons that don't have to do directly with Canvas.
