Question titles in question banks

When I create an essay question in a question bank, the question is marked as "unanswered," even when I put an answer in the "General answer comments." I'm not sure why it's flagging that at all, but the problem is that "unanswered" then gets added to the beginning of the question title (as is shown in the second image). This, besides being an annoyance, also changes the (alphabetical) order in which the questions are listed when I am selected them to go on a quiz or test.

I may be wrong, but I believe you are using classic quizzes.  In that case the first picture looks like the results after a preview which if you do not answer the question in the preview mode, you will see unanswered next to the question.

For the second picture, I know I have seen something like that but I cannot remember (or do it now) what/where, but what you are seeing looks like prefilled text at a point that was not deleted before typing in the question name.

Yes, it does look like an unanswered question from a deployed quiz or from the preview of one. But it's just the way that the questions are showing up in a question bank. (And both screen shots are of the same question in the question back. One is just in edit mode and one isn't.)

It is, for some reason, filled text, but it's not prefilled text that was there before I put in a question title. I put in a title, "PPF-06," finished creating the question, and saved it. What I see in the question bank is what's shown in the first screenshot. If I go back in to edit mode, I have what's in the second screenshot.

Well that is interesting.  I have not seen the type of behavior you are describing.  Hopefully someone else has an answer.

@GRE6 I apologize you are seeing this behavior. I tried recreating this issue, but was unable to. I would recommend reaching out to our support team in order to see if this is an intended behavior. How do I contact Canvas Support? 
I would recommend you provide a screen recording of the behavior if possible when you reach out to support, and make sure to include the URL in the screenshots/recording.
