Text input field in five-question essay quiz not showing

I created a quiz with five essay questions. Regardless of which question is #4, that question will not show the text field.

I have tried everything: different browsers, different computers (all Macs), cleared cache and cookies, and ensured the question text is unformatted. It's always the same result. The text input field for the fourth question will not show.

I am puzzled. It doesn't really make sense ... 

Any thoughts? Anyone?

Have you used this quiz before in previous courses?

Is it a Classic Quiz or a New Quiz?

If a Classic Quiz, are you using question banks?

If a New Quiz, are you using item banks?

If a New Quiz, was it ever previously a Classic Quiz that was migrated?

Knowing this information can help us try to duplicate the scenario.

I was finally able to fix the issue. Creating the Quiz from scratch and resetting the ’Student View’ did the trick!
