Why do I get a "failed to create a new item" message when attempting to upload an mp4?

I am attempting to upload an mp4 into a module and i get a message that reads "Failed to create a new item". Why is this? Any help is appreciated. Thank you.

I would consider uploading it to YouTube as unlisted so only people with the link can see it. Then you can take that link and add it into Studio in Canvas and then from Studio you could move it into modules. Just a thought!
