Duplicated Outcomes

The outcomes have duplicated and are used for several standards.  How did this happen so I can avoid in the future?

This has also happened to me and is driving me nuts. The quizzes that are associated with the outcomes are also duplicated. I hope that this issue can be solved soon!

Question:  Do you manually add Outcomes to your courses?    We are seeing some of this as well.   I'm wondering if it's happening with teachers Share assessments with each other (that have outcomes attached), if they also have manually added their own outcomes to their courses.
