How to set up verification / attestation page after content is reviewed, and collect the data.

Hello,

Non LMS user (until now) here trying to set up some content to distribute. I need to create an "attestation" page, where the student can check a box that they understand the content they reviewed and their responsibility of understanding the content (its compliance content, FYI). I would also need to be able to collect this information via csv to cross reference.

Is there a way to have a check box and a field, both of which are collectible, via a page in my Canvas workflow?
