Require a score on a NEW quiz to finish module glitches

We added New Quizes to the middle of modules. We set it so students must sequentially proceed through pages, then get a 8/10 on the quiz to continue and then do two assignments.
About 10% of students get stuck on the quiz. The quiz grade is not seen in the gradebook but you can see that they did the quiz well.
Then the quiz blocks them from proceeding. I think the Submit button at the end has to be pushed..
Has anyone seen this error or what may be causing it. It is new. We checked browsers. It seems to be random as no one browser is causing it.
