interactive form or document

I would like to upload a document that students can fill out.    Is this possible?

How do I delete a student's quiz score and attempt?  The student submitted without finishing the quiz.  I need to delete his attempt and score and have him retake it.

I can't get a gif to move on my dashboard course card. Any suggestions?

The feature is on in settings.

In Quizzes Next, am I able to upload a document such as a PDF or WORD file as part of the question?

I am able to copy and paste text, but I don't see a place to upload a word or pdf file as part of the question.
