Sorry for the delay! I didn't receive a notification that you had replied to my post.
I had created a Formstack form that captures the student's name, date, journal number (free form field), and a predefined drop down list of the areas they needed to record field hours. This populates a Google Sheet tied to our Formstack account. The first tab on the sheet captures the data inputted into the form and the second tab is a graph that can be used to easily determine how much was logged per activity.
