What causes cosmetic duplicate apps when updating key/secret?

I build courses using existing course cartridges. I then need to update consumer key/shared secrete for the LTI apps. Yesterday, the apps appeared to duplicate when I updated the first app. Opening the course in an ingonito browser showed only the original apps. Refreshing the browser in the original tab also removed the duplicates. 

 

Is this a known issue, and is there a reason this started to occur yesterday? What is the solution to prevent this in the future?
