Individual Education/Learning Plans - Anyone doing this in Canvas?

We have embraced Canvas across our school and Canvas is our source of Teaching and Learning information as well as our assessment and reporting platform. As part of this, we would love to find a way that we could have an individual learning plan attached to a student that goes across all of their courses in Canvas. I have tried using Faculty Journal but I need a little bit more functionality than just plain text and I am keen to hear about how others differentiate learning for students of differing abilities in Canvas.
