We've had that happen a lot. I usually put a ticket in on it, but it normally seems to clear up on its own if you just keep trying. I've also seen it create the assignment in PowerTeacher Gradebook but not sync over scores, deleting the assignment in PTG and hitting sync again worked that time.
One of my biggest gripes with Canvas's Grade Passback feature is the lack of admin facing reports on it. Nothing drives me crazy more than having an issue and not being able to do anything about it. I wish it had some logs that I could look through to at least know myself what the issue is instead of just passing it off to support. It makes me feel quite helpless.
I also put in a feature request awhile back for validation tools, but I think it got pushed to the bottom of the vote list... Basic idea was to have a button you could push that would check the grades in your gradebook against the grades in the powerschool gradebook and alert you if something doesn't match. For example if everything matches up, green all the way across the board. If some didn't sync, make the row red. With a mouse over warning on each to show you what went wrong, and a button to push to re-sync the grades that didn't push over.
