Quiz Regrade Issue

There is an issue we have noticed when regrading or adding points to a quiz that we had never seen before last week.  When regrading an autograded multiple choice quiz or when adding fudge points, we have found that the scores of students who received the regrade or fudge points have a Q in the gradebook instead of their score.  Students not affected by the regrade/point addition have their scores correctly reported. 

 

When you click on the Q in the gradebook, the score is updated, but this seems like a really inefficient way of updating (especially since we usually have 250-300 students in each course).

 

Is there something we are doing wrong or is there a more efficient way of updating the gradebook to reflect the changes in the grades?
