My speedgrader tool bar does not have a comment button, is there a way to fix that?
There could be a couple of reasons for this. First, make sure that the files that were submitted are supported by Crocodoc. Canvas currently supports .doc/.docx, .ppt/.pptx, and .pdf Crocodoc file formats.

Next, the comments feature is not available if you are using the Peer Review feature in assignments.
The comment/annotation options are not always available in the Speedgrader, it depends on the format of the student's submission. If a student entered their submission into a text entry box in the assignment then the teacher won't be able to add comments, the option is only available if the student submitted by uploading a file. Additionally, even for file uploads the comment option only appears for certain file types. If they uploaded a doc/docx or PDF then you should have the comment option, but it's not available for plain text files.
If you never get the comment option even when the student has uploaded a compatible file type, check that you're using a supported web browser:

I have a situation where the document is a .docx file format. It was a group submitted paper (1 of 5 groups). The submissions for all of the other groups, the faculty commented in Speedgrader, but the this group's paper didn't have the comment buttons in Speedgrader. Anytime this document is accessed in Speedgrader, there is a red comment at the top stating "Crocodoc is still processing this document" -- it's possible to close this comment, but the next time the paper is accessed, the message is back.
Since we've ruled out the file format, do you have any other suggestions?
There have been some general issues with Crocodoc recently, which I believe Instructure have now fixed but if your document was submitted during the period that the problem was occuring then this could explain why it's still not displaying. I suggest logging a support ticket with Instructure - give them as much detail as you can about the assignment and which submission is affected, then hopefully they should be able to get it processed for you.
