How do I download a form, fill it out and submit it to a teacher?

Thanks for posting your question! I'm sorry it's taken a while for us to reply. I'm not sure how your instructor's provided the form for you to download.

If it's a Google Doc or a Microsoft Office 365 document, you'll want to check out How do I submit a cloud assignment with Google Drive? or How do I submit a cloud assignment with Microsoft Office 365? 

If your instructor linked to a file they've uploaded in the course, you can checkHow do I view course files as a student? for information about accessing course files, and then you can check How do I download a single file as a student? for information about downloading the file. From there, you can complete your assignment, and then you can visit How do I upload a file as an assignment submission in Canvas? for information about uploading the file as an assignment submission.
