[Reports] Include readable names when generating reports

Problem statement:

When running reports, a number of fields use SIS or Canvas IDs, and these are not always readily readable when consulting reports

Proposed solution:

Include the name of objects when generating reports to permit humans to view and digest the data presented easily and simply. For example, the Admin provisioning report includes most of these, but omits the name of the account, leaving the subaccount admins in an amorphous blob that needs to be worked on further (as our account_id is the org unit code, not its name, in order to synchronise with our SIS) - current columns are: admin_user_name canvas_user_id user_id canvas_account_id account_id role_id role status created_by_sis
