Student annotations hidden in doc viewer

Hello,

I have an issue with students submitting a document with annotations or comments added. Sporadically, I will get a document with hidden comments. They are not visible until the mouse scrolls over just the right spot, then a yellow stickie note comment will show. This does not happen regularly but there have been times that a student does not get credit and then comes back to show me their comments.

What could be the issue here?
