Different students have different questions in an assignment, both between various students and different to the questions in assignment setup/edit

I have an assignment in a course which has had a single set of questions for over a year now, and only recently upon grading a secondary attempt for a student have we detected that the questions are in different order for some students and in most cases different to the order in the assignment design/edit screen. This has only just occurred within the last week and all students who have successfully completed the assignment previously have the correct answers in the correct order, it seems to only be applying to second or third attempts or to students who had not yet commenced the assignment prior to this week.

In some instances the questions have changed order, and in some instances some questions have vanished... does anybody have any ideas what the cause could be or had a similar situation?

Thank you for posting on the Instructure Community!

When you are referring to "assignment questions", do you mean questions from a graded / ungraded quiz or survey? If so, are you using Classic Quizzes, or New Quizzes? In New Quizzes under the Settings tab via the Build section, you are able to toggle on Shuffle Questions and Shuffle Answers. Each time a student takes a the quiz again, it will randomize the order each time, which will result in your students having a different question order than other students.
