I am an Instructor: Posted grade value hidden in grade book?

I have graded an assignment, made it visible to students in speedgrader, and posted it.   When I review the grades, some grades are hidden by an icon that looks like a piece of paper.   If I hover over it, it will show the entered grade.  How can I be sure, this grade was included in their total?

If the symbol you are seeing looks like this: , as mentioned in that link, it means that the submission may not have been graded yet (either because you haven't graded it, or you have but then removed the grade, or there was a resubmission).

So you can fix it by giving it a grade. If it is a quiz, there's likely one question (or more) still needing a grade.
