I have gotten them in the past when I have deleted someone and then tried to add them back with a different login. Our SIS managers go through and find duplicate entries and take actions to correct it, sometimes they end up keeping the new ID and reusing the old login or they may keep the old ID but assign it a new login.
What I've had to do was to add the user back with the old login so that I could change the SIS ID through the web interface. Then I can go through and reuse it.
If you're not sure what login was attached to the old SIS ID, then you can run the provisioning report and include deleted objects to find out.
