I want students to fill out a form with information about themselves on Canvas. How can I do that?

Hi,

To do this I use an ungraded survey (found under Quizzes), then use Essay or MC questions to collect the info I am after (e.g., course, any questions for first lecture). For large classes I'll keep submissions anonymous, however for smaller classes I collect names so I can follow up any specific questions or issues directly (however I add a note in the instructions that names are being collected).

I require a signed student handbook for every student. I make this an assignment and add a fillable PDF as a file link. The students open the file fill it out, save it to their computer and then upload it to the assignment. Be careful to limit the file types for the upload as a lot of younger students will use iPhone s to take pictures of the fill out forms and upload large file pictures that become difficult to download and save.

I used to include a word document for students to fill and upload. I had to then transcribe everything into excel for easy storage and reference. I feel an ungraded quiz is a better way to collect student information.
