In general, when you have a discussion to answer, you have to goto the Discussion page in the course and read the discussion prompt the professor has left for the class. That's where you'll see more information about whether they want facts, opinions, multiple answers, APA formatting and so forth.
All answers to the discussion due at the end of the week means that you have to following the discussion prompt and post your response before the end of the week. In the case of "Using the Nursing Process" there is probably more detail about what the professor wants you to answer.
You do not have to answer the questions of all your classmates unless that is in the instructions on the Discussion page. Considering that your syllabus says the initial discussion post, generally means you will probably have to reply to a few other students initial comment. If you are familiar with Facebook, replying to a classmate is like replying to the comment that someone made and it starts an offshoot thread.
