[ARCHIVED] Quiz question format for a Chart

I'm creating an exam (quiz) in Canvas and I need to add a question where students fill in a chart, as shown below.  They have to identify the tense and conjugation of verbs and then divide them into morphemes. What's the best way to adapt this to a quiz in Canvas?  Fill in the blank?  Or should I just do a file upload question?  Thanks.

I use charts just like this by pasting it in and then using multiple fill in the blank. I haven't found a way yet for this to work with the New Canvas Quizzes, though.
