Here is a screenshot from the teacher view of the application configurations in the course. This is where we are stuck. When you click on the settings wheel to add placements it will not allow to place in the Canvas course.....? So the app is in the course but not showing up any where in the course because we can't place it.

I am a Canvas administrator for our school's Canvas environment, and I've had a bit of experience installing LTI external apps at the account level for our instructors to use in Canvas.  The cog wheel icon that you are clicking on is not something where you'll ever be able to specify where an LTI app is installed within Canvas.  I'm not completely sure on this, but I believe most of the time, the decision on where the LTI is placed is actually left up to the developer of the LTI ... not the end user.  I'm not a programmer, so I'm not sure if any of this can be configured with the LTI's XML file or by some other means.  You'd need to reach out to the folks at Scootpad to see if this was even possible...it may or may not be.
