Making a quiz in Canvas

I am trying to make a quiz in Canvas for the first time.  I keep trying to add a 5th question.  I save it but it does not show 5 questions on the preview.  Am I limited to 4 questions or less on Canvas quizzes?

Not to my knowledge.  You should be able to make as many quiz questions as you need for your students.  Have you tried reaching out to Canvas Support to see if they might be able to help with this?  How to contact Canvas Support

I just created a 25-question quiz.  It let me just keep adding one after another. I don't know what could be limiting it for you.
