[ARCHIVED] Missing quiz

I have an instructor whose quiz, which all students took and received grades for, has completely disappeared from her course. When we tried the undelete feature, the quiz isn't even listed in the history. This probably because she didn't delete the quiz. We don't know what happened or why it has disappeared. 

What do we do to reset her course to a previous date or recover a quiz that wasn't deleted but is no longer showing?

 @jill_mcdonough ‌, our members can hypothesize as to what happened here, but the quickest way to get this resolved is for the teacher to initiate a ticket with Canvas Support directly from the quizzes page (How do I get help with Canvas as an instructor?‌)
