CCSNH Screening App for students, faculty & staff

The message contains both a QR code to quickly download the screening app as well as the users’ SIS ID (College ID) to register themselves with the app.

CCSNH Screening App* is an easy-to-use mobile app for students, faculty, staff and visitors to quickly report travel and health information prior to each on-site visit.

The app is available free of charge through the Apple Store and Google Play – search for CCSNH Screening App and click Install. Or download the app directly by scanning this QR code with your mobile device.

Once downloaded, register by entering your College ID (displayed below) as the username. A one time verification email will be sent immediately to your college account. This instructional guide is available for signup assistance.

Don’t have a mobile device? From a web browser, go to http://healthreport.ccsnh.edu/ and complete the screening process from a PC or a laptop. Simply take a screen print of the upper portion of the Dashboard showing name and health status for the day!
