This behavior has been resolved and deployed to the production environment as of 7/02/25.
When "One Question at a Time" is enabled and "Allow Backtracking" is disabled, media files with an instfs_id and access_token show a "Refused to Connect" error for students.
Media files should load for all users.
Disable “One question at a time” option or enable “One question at a time” option and Allow Backtracking.
Prerequisites: A new course with new quizzes enabled.
Create a new quiz with questions where you have a video or audio file uploaded from your computer or recorded in browser using upload/record.
Wait for the background job which converts links to inst-fs style links to update the media_attachments_iframe link in the iframe to add the instfs_id and access_token parameters to the link.
Under quiz settings.
enable "One Question at a Time"
disable Allow Backtracking.
Act as student or use preview.
Notice that audio and video file return 'Refused to Connect' error
