The issue appears to be with "vanity" URL's or whatever you want to call them. I pulled your extension from Git and modified a few lines, ran it locally and was able to make it work for canvas.iastate...
Ran it quick on one of our courses and it appears to be working, thanks again for the hard work! That opens it up a bit for everyone now
