How do I create a new global navigation link?

Account admins can customize the help options that display in the Help button of the global nav and make other changes in that menu by following the instructions in How do I customize the Canvas Help Menu for an account?  Adding items to the global nav is associated with custom CSS and JS as further described in How do I upload custom JavaScript and CSS files to an account?
