The highest score to keep option has been selected, but the default grade is still the last submission. This issue was raised a few years ago and i am wondering if it has been resolved? Or is there a recommended solution. Individual adjustments is not a viable option due to large numbers.

I just tested this in one of my sandboxes, and it worked as expected. I then tested by hinking my quiz then regrading it, and it still worked.
I can only suggest submitting a support ticket. Open Canvas, click "Help", then choose "Report a Problem".

I actually just experienced this issue as well.
A student completed a quiz that allows two attempts for the first time on August 1, 2021, well before the due date, and the grade was not marked late and earned a 9/10. Then, on August 8th, the student completed their second attempt, which was 4 days late with a Late Grade Policy of 10% lost per day, (-4 points) and scored an 8/10. The Grade Book showed a 5/10 score for the student.
I experienced this a couple of times in the summer - but I had small classes, so I didn't think it was an issue. I now have ~800 students - it's most definitely an issue. It's supposedly a known issue, and the engineers are working on it, but there's no word on when they may resolve it.
