[ARCHIVED] How to add an evaluation tool to a course?

I'm building a learner-paced nursing professional development course in Canvas that will be added to Catalog for access by RNs anywhere, anytime.  I need to add the evaluation tool, and I'm not sure how to do that.

Sample of elements to include on the evaluation is attached.  Thank you!
