How do I download images into quizzes? Do they have to have specific file name?

I think you mean "upload" instead of "download". Downloading an image usually means you are saving an image from the internet to your computer. Uploading an image means that you taking an image file from your computer to place it somewhere on the internet...such as in your Canvas course. Now...to your questions. Images don't have to necessarily have a specific file name, but for organization, you may want to name them so that you can identify what image they are...or organize your images into folders within your Canvas course.
There are a couple options for you as far as uploading images to your quizzes...keeping in mind this is only available to those question types that use the Rich Content Editor (which is most question types):
