[ARCHIVED] Logging In To Complete Course Evaluations

I am a dual credit student at Cisco College. Around this time is the time to complete the course evaluations for each of my three courses. However, I am being asked for a username and password that I do not have to be able to complete the course evaluations. I consulted a few of my other classmates who have already completed their course evaluations. They explained that they did not need to log in to anything to complete their course evaluations. So, I am curious to find out what I can do to be able to complete my course evaluations.
