One assignment has the same exact submission for every student in my class.  

How do I fix it?

How do I get rid of it?

Can it be refreshed?

What can I do?

Did you by any chance set up the Assignment as a group assignment and then add everyone to the same group?  It sounds a little like that because with a group assignment when one student from a group submits, it gets submitted for everyone and will show that way in speed grader.  

You can try to change the settings on the assignment to remove the checkbox about being a group assignment, but I'm not sure it will let you change that after submissions have been made.  You might need to create a different assignment without the box checked to make it a group assignment, and ask students to submit there instead.
