I think it might depend on your process. Two different things that I can think of could be at play. Where are questions being created and where are you viewing question banks?
CREATING:
In classic quizzes, if you create questions within the quiz itself, they are stored in the Question Banks in UNFILED QUESTIONS. (see attached image) If you begin a quiz by creating the question bank first, then building the quiz using FIND QUESTIONS from the bank, you will see the named banks in MANAGE QUESTIONS BANKS. It's pretty easy to organize your unfiled questions after the fact by creating a bank and moving the one/some/all of the questions into it.

VIEWING:
If you can see a question bank when building a quiz, but not when managing question banks, it could be that the bank is in another course. FIND QUESTIONS allows you to access question banks across your courses. MANAGE QUESTION BANKS, to my knowledge, only shows the banks that "live" in that course. The 2nd attached image shows the bank view from FIND QUESTIONS. Clicking the circled link will take you to MANAGE QUESTION BANKS.
New Quizzes allows you to bank questions as you create them within the quiz. If you're not finding a bank for a particular quiz, you can go back to the quiz and file the questions.
