Why won't my external tool work?

Have created an assignment using and external URL. Student open and edit the office365 document in the browser and submit their response. Some students are able to do this successfully, other are not able to access the document. Students have had success and failure while using a PC and iPad. Because some students have been successful, I know I have is set-up correctly, so I have no idea why it works or some and not others. I am open to ideas....
