[Credentials/Badges] Students able to request name change

Problem statement:

As a student, I want to be able to request a name change on a credential that has already been issued to me. Changing the preferred name in the user account doesn't update credentials that have already been issued. The student currently has to contact their administrator to request a name change.

Proposed solution:

An option when selecting to share/print the credential to change the name that is displayed.

Problem statement:

When a student's name is incorrect on a credential (for example, they have used their maiden name or they are now divorced since the original issue), there is no easy way for them to correct it without emailing the course delivery team who then need to re-award the badge.

Proposed solution:

Allow students to submit a name change request for their credential. This request would then be sent to issuer level admins to approve or decline. If approved, the name change would update on the credential. This will make it much easier for students to update the name on their credential without having to struggle to go through emails.
