[Discussions] Disable Document, Media, Images Upload

Problem statement:

Canvas provides a setting to disable attachments in Discussions, but left the Document Upload option available in the formatting toolbar. As such, students can still attach files.

Proposed solution:

When this "Let students attach files to discussions" setting is disabled, disable the upload document, media, and images buttons to prevent attachments entirely for discussion posts by students.
