[Gradebook] Export with due dates and assignment group rules

Problem statement:

At the end of every semester, we need to turn in the grades, along with all due dates and grading rules. The gradebook export setting records the grades, but not the due dates or the assignment group rules.

Proposed solution:

Update the export settings to include a new row in the header. For each assignment include both the due date and assignment group in that row. For each assignment group, include the assignment group rules (i.e. drop lowest 2 scores) in the new row. For the total score, include any penalty associated with the late policy.
