[New Quizzes] Remove autocomplete from Quizzes

Problem statement:

Autocomplete creates confusion for students filling out quizzes that use input boxes. In some cases, this could even facilitate academic dishonesty because the students will have the answer as a suggested response. There does not seem to be any benefit to keeping autocomplete.

Proposed solution:
