I also use Canva, it's a very robust online tool. They have a free for educator account which unlocks their image library and other features. It's quite a deal.
Create and Collaborate | Canva for Education
Once you login, at the top right you can create a new canvas with the recommended pixel dimensions for a course card.
For best practice, images should be approximately 262 pixels wide by 146 pixels high. From:

I'm not sure if you are an instructor or a student, but adding an image to your course card on the dashboard is only a function that instructors can set. When instructors add an image to the course, it shows up for all students enrolled in that particular course. Here is a Guide on how you can do that, if you are an instructor:

I actually know how to upload an image, etc. I'm looking to design a custom image put on the course tile. Say develop it in PPT, I know this can be done, just can't remember how to do it.

@hbrown5018 If you are going to use PowerPoint, you could design a slide the way you want your image to appear and then you can save that slide as an image. Here are instructions on how to do that: https://www.howtogeek.com/707583/how-to-save-a-microsoft-powerpoint-slide-as-an-image/
You could also look into using Canva to create your image and download it as an image to use. After you design a custom image for the course title, you might consider designing some additional elements for your course like a course banner: https://www.canva.com/
