[ARCHIVED] Speed grader innacurate?

Myself and a fellow instructor have noticed that the speed grader is not always accurate. I have had a student email me a photo of their quiz stating they got the answer wrong, but then shows the correct answer which is the answer they selected. Anyone else running into this? I am also wondering if there is a way to be notified when a student takes a quiz. The speed grader just enters it without notifying me, but I have to have this information in order to enter it into their E-TAR,
