[OPEN] After uploading/recording media in a submission the player can become unresponsive in speedgrader

Description
If your initial embedding of media (audio or video) fails to properly replace the placeholder data that gets injected into the RCE after submitting the media popup, attempting to record a new media recording will usually work, but will have been injected within the prior placeholder data resulting in the media player becoming unresponsive for teachers in speedgrader.
Expected Behavior
Identify why the media recording process seems to fail on the first attempt to embed it so often.
If placeholder data exists in the RCE when attempting to add another properly scrub the data out before injecting the new media recording.
Workaround
No workaround exists at this time.
Steps to Reproduce

Create a New Quiz with an essay question

As the student record/upload some content

Notice the initial upload/record doesn’t show up

This is easier to reproduce with a longer recording, but especially so if you click out of the RCE after clicking submit on the recording.

Refresh the page and try again

Notice this time it shows up, but if the cursor was inside the initial span tags it becomes unplayable after saving
