Adding Assessment to Tracker

Why do I keep receiving this error message when trying to add an assessment to my tracker? How do I fix it?

Great question! I would encourage you to submit a support ticket for this.  The team will need to know the name of the assessment and the name of the tracker that you are trying to add the assessment to.
