Deleting Questions and Reappearing

I have teachers that are cloning and fixing issues on questions and deleting the incorrect ones. HOWEVER, the deleted questions reappear in their item bank when they refresh their screens. HELP!

When an item is already used in an assessment and attached to a tracker with student data, it will remain in the item bank to preserve data integrity.  See the following information in Item Authoring about Item Versioning.
